The External Saphena Geometry. Surgical choices.

A thorough study of the geometry of the arc of the external saphena (ES) provides some specific findings.

As to the collateral's of the arc, it is useful to point their number, while as to the collateral next to the popliteal vein it is important to note:

• its flow (low, middle, high)
• its flow direction, toward or outward the ES
• the velocity gradient (sudden or gradual), comparing the flow to the ES
• the angle to the arc (acute, recto, obtuse)
• its position under, over or trans the fascia
• its calibre, compared to the one of the ES
• the length of arc downward the collateral

Also in the ES arc it is important to note:

• its tortuosity or dilation, reaching sometimes the aspect of a cavernoma
• the pulsatility (during breath or muscle movements)
• stasis at rest.

These findings can help in assessing the risk of thrombosis, so guiding to the choice of the most effective surgical procedure.

The importance of the last collateral of the arc is evident when this vein is used as a washing vessel for ES arc.
